What you will need to bring to your appointment:

If you have recently been married or have legally changed your name you will need to provide an original Certificate of Marriage Document or Legal Change of Name Document.

For the Secure Certificate of Indian Status (SCIS) Card

The SCIS will be mailed to your address, and be valid for ten (10) years for adults and five (5) years for children.

IMPORTANT NOTE:
SIN cards, photocopied ID, bank cards, credit cards, and damaged or mutilated ID are NOT acceptable forms of ID and will be rejected.

3. Name - Linking Documents

You must provide two (2) identical and unaltered Canadian passport-style photos, taken within the last twelve (12) months of the date of your application.

At least one (1) piece of valid government issued photo ID that contains your name, DOB, photo and signature.
